Having the member present for an in-person interview is expected. The next best option is to complete the interview by phone with the member
You may complete the interview using a printed copy, however it is not complete until it has been submitted into the system
Copies of all submitted NC-TOPPS interviews must be included in the member’s service record
When a member changes providers, the QP at the new provider agency will notify the MCO and the MCO superuser will change the NC-TOPPS to the new provider QP
A Release of Information form must be provided with the request when a member’s provider agency changes
The member’s QP at the provider agency is the person responsible for ensuring that NC-TOPPS interviews are completed
