Providers must ensure an appropriate Release of Information (ROI) or consents are in place when sharing information and completing NC-TOPPS
The primary provider agency may share a member’s PHI only with those provider agencies explicitly named on the signed consent form
When requesting NC-TOPPS be switched to your agency, you must provide an ROI. The ROI must have the name of the agency who is providing the information (Vaya Health), the name of the agency receiving the information (Your agency) and what information is being sent/received (NC-TOPPS)
The ROI form must be renewed at least annually
