Common Name Data Service Numbers (CNDS#)

The CNDS# is a unique identifier that is generated in NC Tracks. You can find this number in NC Tracks and in Alpha/MCS.

If a member has Medicaid, the Medicaid number is the CNDS#

You will need to provide all of the following information when submitting and email request for the CNDS#:

• First and last name (no nicknames)
• Date of birth
• Social Security Number
• Gender
• Race/ethnicity
• Language spoken
