Transition to Community Living (TCL) TCM Training

This course provides a foundational understanding of Transitions to Community Living (TCL), community-based living, and supportive housing in North Carolina. It also provides a foundational understanding of the TCL Local Barriers Committee process. Care managers will use this knowledge to recognize when members may benefit from diversion or housing-related support, collaborate effectively with Transition and Housing teams, and support member choice and stability in the community.
