This course provides Care Management with an overview of developmental transitions for individuals with Intellectual and Developmental Disabilities (I/DD) and Traumatic Brain Injury (TBI) from birth through age 21. It focuses on how needs, supports, and services change across key life stages, including transitions from early childhood through primary and secondary school settings and into adulthood. The training also highlights the importance of planning for the transition out of school-based services and supporting lifelong independence, education, employment, and community involvement.
